About the role:

We are looking for two highly motivated Assistant Team Managers to join our Hospital and Reablement team, who enjoy working at the interface of hospital and community teams, to ensure we deliver the very highest quality integrated health and social care and show passion in supervising therapy and/or social work staff to help develop their professional practice and career development

You will be representing Haringey as a senior member of the Integrated Discharge Teams across North Central London, coordinating a multi-disciplinary team of professionals in the safe discharge of people out into the community, utilising the principles of Discharge to Assess and promoting Home First.  You will provide expert advice and guidance for complex social care and work closely and collaboratively with other teams and organisations to aid in complex decision making of referrals received from the acute trust and rehab units.

You will be expert in giving others advice and leading by example to ensure that support plans reflect integrated health and social care, are well written in plain English, and embrace a recovery and strengths-based approach.

As a leader in the Reablement team, you will also assist the Team manager to ensure that therapists and social workers are supported in their practice and offer a reflective space and professional supervision, monitoring Key performance indicators and outcomes.

You will lead and be competent accessing and using Council resources and information systems,  including MOSAIC and Community Care Inform, and systems that support learning and development in Haringey

About you:

You will be a qualified Occupational Therapist, Physiotherapist or Social worker, with the motivation and skill to bring out the best in your colleagues.

You will have a sound knowledge of legislation that underpins health and social care for service-users with care needs and will have expert problem solving and analytical skills to be able to interpret clinical information and make key decisions on discharge pathways.
